Chaperone Licences
The law requires that if children of compulsory school age are taking part in performances, filming or modelling assignments they are supervised by a registered chaperone, unless they are under the direct supervision of a parent or registered tutor.
The checks associated with becoming a chaperone are made by the Education Welfare Service prior to issuing the appropriate license.

Please note you are required to submit the following with your application:
- Photograph
Two recent passport size photographs
- References
Name and address of two referees who will be asked to provide a written reference
On receipt of the above documents, they will apply for written references. Once they have been received they will contact you to arrange an interview. If you are successful at the interview stage they will have a Disclosure Application Form for the Criminal Records Bureau (CRB form) sent to you. When completing the form please pay close attention to the following and the enclosed CRB form guidance notes:
Section B13 must state CHAPERONE - VOLUNTEER
Section B14 must state NORTH EAST LINCOLNSHIRE COUNCIL
Section Y8 must be completed
Please allow at least eight weeks for the CRB check to be completed by the Home Office.
When Education Welfare receives confirmation of a clear CRB check they will issue a licence. The licence will be valid for three years.
