Smoke Nuisance Record Form

Only record incidents which give rise to problems in your household. Complete entries when the Smoke occurs, or as soon as possible afterwards. If the smoke occurs in short bursts over a period, simply record the approximate time that it was first noticed and the time when it was last noticed: Include an estimate of the number of occurrences and their typical duration together with your description of the Smoke. Please ensure all of the mandatory sections identified by a red asterisk are completed.
