How do I refer someone to the Positive Futures Scheme?

Should you wish to refer a young person to Positive futures you should read the referral form and criteria to ascertain if the young person would fit the aims of the project funding. You then need to obtain consent from the young person and their parent/carer and complete the referral form as fully as possible. The form can then be faxed to 01472 324819 or posted to the Positive Futures Team at:-
