Electronic Guide to Local Health and Adult Social Care Services

"Your Guide" helps you choose the right health and adult social care services for you and your family. It has been produced by your local NHS and includes important information and contact numbers for health and social care services in North East Lincolnshire. This includes details about local GPs, care homes, pharmacists, dentists and out of hours services for urgent care.

A3 - For All Social Care Enquiries

A3 is the main route for the general public to access information, advice and guidance for adult social care and urgent and non-urgent community health and social care services, including adult social care, occupational therapy, physical and learning disabilities. (This does not include access to GPs or district nursing teams). If you, a family member or friend require or think you may require social care services please contact the A3 team on 01472 256256.

Free Breast Cancer Screening

All women aged 50 to 70 are invited to have a FREE routine mammogram every three years. If you receive an appointment please attend - it could save your life. Please see the poster and flyer from the document downloads list for more information.

Visioncall

The Care Quality Commission is the health and social care regulator for England. They look at the joined up picture of health and social care. The aim of the Care Quality Commission is to ensure better care for everyone in hospital, in a care home and at home. Arthritis Care exists to support people with arthritis. They are the UK’s largest charity working with and for all people who have arthritis. They are a user led charity which means people with arthritis are at the heart of their work – they form the membership and are involved in all of activities and direct what Arthritis Care do. NHS Diabetes

NHS Diabetes works to raise the quality of diabetes care in England by supporting and working with the healthcare community and people with diabetes. The team’s role is to work with health providers to improve the delivery of diabetes care in England across all areas of healthcare. In partnership with people with diabetes, they help develop and support new guidelines, standards and systems designed to improve care, and then encourage the widespread implementation of these new initiatives. National Osteoporosis Society

The National Osteoporosis Society is the only UK wide charity dedicated to improving the diagnosis, prevention and treatment of osteoporosis. The charity was established in 1986 and has since grown into a well respected national charity with approximately 25,000 members and over 50 members of staff.
