Leaving care can be an exciting time - your chance to become more independent and make your own choices about your future.
You'll find you need information and support to help you make the right decisions for you.... that's where the 16+ Service can help. We'll listen to you and involve you when planning what services can be provided for you.
At the moment you may be living in a children's home, with foster carers or in another type of accommodation. You and your Social Worker are probably planning your move towards independence and transfer to the 16+ Service.

To make sure you'll have support if and when you need it, a Case Worker will attend your final review before your 16th birthday to discuss which services you require and for how long. We'll then draw up a Pathway plan and will be there to support your move and for as long as you need us until you're 21.
Your Social Worker will still be the Key Worker until your case is transferred to the 16+ Service on your 16th birthday. However there will be a gradual introduction to the 16+ Service before you turn 16.
When your case has been transferred you will be given a named Case Worker to offer you support during extended office hours and at weekends, but if she/he isn't available and you need help straight away there'll be a duty worker available.
We'll also help you develop your own support by maintaining relationships and contacts as well as making new ones.

Finding a place to live in Grimsby and Cleethorpes is sometimes not easy. Case Workers are here to help you.
There are different types of accomodation and, depending on your Pathway plan, suitable housing will be sought for you. The options available are:
- Supportive Lodgings
- Shared Housing
- Hostels - offering different levels of support. Staff are available to help you and meals may be provided but you'll be expected to live by the rules and to respect other people living their.
- Independent Living - if you're ready to be living on your own. Your Case Worker will help you find a home which might be in a bed-sit, a room in a house or a 1 bedroomed flat.
