We work really hard to provide you with up to date, accurate information about services and activities for children and families. You can search our directory for information about activities that are taking place in your area. We also work with services in and around the surrounding areas to ensure that you can find a service that you may need at any given time.
We constantly work on the information provided, the way in which you can search for information and we welcome all feedback about the directory from you! What does the Health Visiting Service do?
- The Health Visiting Service works mostly with families with children aged 0-5 years and the communities in which they live.
- The key aim of the Health Visiting Service is to promote, maintain and improve the health and well being of children aged 0-5 years and their families.
- Working with others enables staff within the Health Visiting Service to ensure the needs of service users are met in the most appropriate way. Examples of key partners are GPs and children's centres.
- Provides access to high quality support and guidance for all families with children aged 0-5 years.
- Signposting is one of the key functions of the Health Visiting service.
Who is in the Health Visiting Service?
- Health Visitors:
These are all qualified nurses who hold a secondary qualification at degree level, in health visiting. This equips them with the skills and knowledge necessary to work with individual family centered health and well being issues as well as wider public health issues.
- Community Family Health Nurses:
These are also fully qualified nurses who are keen to develop their careers within the Health Visiting Service. They will have already developed a range of skills and knowledge before joining the service and support service delivery.
- Nursery Nurses:
Hold a relevant qualification in child development and childcare and have gained further experience working within the Health Visiting Service supporting families under the guidance of Health Visitors.
- Administration Staff:
May well be your first point of contact with the service and provide valuable support to all team members.
From time to time there may be other professionals working within the service, such as student nurses and student Health Visitors. All staff carry appropriate identification or you can ring the general enquiry line if you are unsire about the identity of someone wanting to visit you from the Health Visiting Service.
If you want to discuss health or developmental concerns about your baby or child aged 0-5 years, feel free to drop into one of our well baby clinics.
- Accessing the Health Visiting Service:
The Health Visiting Service is an NHS service, locally provided by the Local Authority, available to all families with children aged 0-5 years, or expecting a baby, who are registered with a NE Lincolnshire GP practice. There is no charge for this service and it is based on need with no referral necessary.
The Health Visiting Service is usually informed by it's health partners when a baby is born, or a child under 5 years of age moves into the area and is registered with a local GP. This information is used to contact parents and arrange appropriate Health Visiting contacts. If you believe that this information has not been received by the Health Visiting Service, please contact us directly.
Families are allocated to the appropriate Health Visiting team depending on where they live and a named Health Visitor is identified to organise their health visiting care.

Early Intervention In Psychosis & Transition Team
The Early Intervention in Psychosis and Transitions (EIT) service provide specialist personal care to young people who are at risk of, or experiencing, their first episode of psychosis.
If you’re between the ages of 14 and 35 and are at risk of – or experiencing – psychosis, we’ll support you with a forward-thinking personal care package that challenges the stigma and misconceptions of mental health.
We believe that psychosis shouldn’t stop you from living a worthwhile, productive and happy life. We aim to prevent psychosis from stopping you living. We’ll get you back on your feet and on the road to recovery as quickly as possible.
You’ll be allocated a care coordinator who will design a care plan with you and, if you agree, your family. We’ll actively listen to you, working collaboratively alongside you to empower you to stay connected, optimistic, build your identity and live a meaningful life.
There are two care pathways within EIT:
The traditional First Episode of Psychosis (FEP) pathway is a three-year period of care targeted at helping you recover from psychosis and lead a productive and happy life.
The At Risk Mental State (ARMS) pathway is a six to 18-month period of care aimed at helping you avoid developing psychosis.
Regardless of which pathway is most suited for you, we’ll work holistically with you and your family, supporting you across all aspects of your life.
We offer a number of therapies and support, including:
Cognitive Behavioural Therapy
Family interventions
Carer education and support
Employment and education support
Medication and physical health screening
Wellbeing groups
Transitions
We also offer support to young people making the transition from child and adolescent mental health services (CAMHS) to adult mental health services, regardless of diagnosis.
We work alongside the local CAMHS team, getting to know the young person and identifying the most appropriate NAViGO service for them. This is usually a six-month service.

About us
Our vision is a society where Deaf and disabled people are valued as parents and can access the support they need to carry out their parenting role.
DisabledParent.org.uk is a website belonging to Disability, Pregnancy & Parenthood, a user-led registered charity that promotes better awareness and support for disabled people during pregnancy and as parents.
Our mission is to provide Deaf and disabled people, their families and service providers with information and shared experience on all aspects of pregnancy, childbirth and parenting.
We recognise the important role Deaf and disabled people play in identifying solutions to their own challenges and that these solutions can be of benefit others. This site shares some of that knowledge and experience that we have been collecting since 1998. And now, this site provides opportunities for disabled parents and their professional allies to share that expertise more directly with others.
We are base in England, and our information is specifically targeted at disabled people in England. However, until 2012 we published a quarterly international journal, Disability, Pregnancy & Parenthood international, dedicated to disabled parenting not only in England but in the wider international community. The journal is no longer published separately, but the content is being integrated into this website.
Our aim is to take experience and expertise from where ever it may come, and to share it wherever it is needed. To this end, our resources, in particular our unique publications, are requested by people around the world.

You can ask a question to be answered by the growing community of users who bring diverse knowledge and experience. You can help others by answering questions, writing reviews, and sharing your experience and ideas.
You can also order our publications: accessible guides on many aspects of disabled parenting. A small charge is made publications to help cover costs. However disabled parents may request to receive publications free of charge by emailing [email protected].

Emergency Dental Services
Emergencies
Are you in need of emergency dental treatment?
Are you in pain due to a broken or lost tooth? Have you misplaced a filling? Do you have an ache or unexpected pain and swelling in your tooth or gum?
Whenever you are in need of an emergency dental appointment then you will want to be seen at the earliest opportunity- particularly if you in pain or struggling to eat properly.
Please call us as early as possible in the morning in order for us to be able to offer you an emergency appointment.
Ways to help until you can make your appointment.
Until you can visit us to get the problem sorted there are various ways in which you can help to relieve any pain and restore function to your tooth temporarily so that you can continue your day in comfort.
If you have knocked out an adult tooth then try to re-insert it into the gum. You can hold it in place by biting down on a clean cloth (tea towel, gauze). If you can’t replace the tooth make sure you store it safely in a small container. Don’t let it dry out- keep it moist by covering it with milk, saliva or a sterile solution (not water). Avoid handling the tooth by the roots and NEVER attempt to clean it.
Provided the above instructions are followed it is often possible to successfully re-insert a missing tooth.
If the tooth is broken but hasn’t fallen out completely gather up all the pieces and store them (as above). Rinse your mouth out with warm water to remove any small pieces. Take a painkiller if necessary.
If you have lost a filling you can temporarily cover it with a small piece of sugar free chewing gum pushed into the cavity. Be careful not to eat on this tooth! Likewise, temporarily re-attach a lost crown with chewed gum (provided it hasn’t broken).
Please note: all of the above suggestions are temporary measures. It is important to visit us at the earliest opportunity for the necessary dental work to ensure that further complications don’t occur.

Everyone aged between 40 and 74 who has not already been diagnosed with any of the conditions mentioned above will be invited for an NHS Health Check.
The check involves having a blood test and a review of your blood pressure height weight and lifestyle with a repeat check in 5 years time or sooner if necessary.

Littlefield is situated in the centre of Grimsby, well known for its past history as the world’s premier fishing port. The fishing industry has been in a steady decline since the 'Cod Wars' of the 1970s and no longer has its own trawler fleet. Grimsby is now recognised as the 'food town' of Europe, processing and freezing more food than anywhere else in the Continent, providing jobs lost through the demise of the fishing industry.
Until 2009 the practice occupied GP owned premises, but in line with the governmental strategy, we are now based in the Freshney Green Primary Care Centre, a state of the art building completed in 2009 by Assura. The development is the result of the combined efforts between Care Trust, Littlefield and two other local practices, Fieldhouse and Woodford Medical Centres, to bring into one building an array of services and provide the best possible care for its patients. The building is part of the old Yarborough Estate Neighbourhood Regeneration Project, driven by the local council with the support of Shoreline Housing Association. The whole area has been renamed and redeveloped to reflect the regeneration and the investment made by the trust and the three practices in the building aims to make it a testimony of the changes for the local community.
The practice serves a population of 5,600 patients and the building itself serves 32,000 people, around a fifth of the total population of North East Lincolnshire. The new premises house a number of different health professionals including district nurses, health visitors, adult social care, social care workers and both secondary and primary care mental-health professionals. Other services available include physiotherapy, podiatry, dentistry, pharmacy, minor-operations suite, smoking cessation, audiology and the local health-informatics training suite. In the very near future we hope to add ultrasound to this list.
We have four Partners and one full time Salaried GP, who are all heavily involved in training. Through our Partnership with HYMS (Humber and Yorkshire Medical School) and the Deanery we now train in-house: registrars, Foundation Year 2, Year 5 and Year 3 Medical Students. We also offer placements to Dutch Medical Students. The Practice has had a female GP since the early 1960s.
The demography of the practice population is similar to Local and National distributions. Although we are based in the town centre our population extends to the villages immediately adjacent to Grimsby and Cleethorpes and includes Patients who live in recognised deprived areas of both towns.
Along with five other practices in the area including Fieldhouse and Woodford we have funded a Company, Yarborough Clee Care Ltd part of the NHS family, employ the District Nurses serving our population. We now also employ one Primary Care Mental Health Nurses and two Health care Assistants specialising in Dementia.
We are a computerised, 'paperless' Practice currently using EMIS Web as our Clinical system.
We are a foward thinking team and have piloted most of the initiatives introduced by successive Governments. The whole team welcomes external evaluation as a way of keeping us on ‘our toes’ and measuring our performance against nationally set standards. We are constantly looking at ways to improve the services to our Patients and actively involve our well established Patient Participation group in service development and changes to systems. We gained the Investor in People Award in 2002 and were re-accredited in 2008, against the new, more stringent, criteria. We also gained the Chartermark for Excellence in customer care in 2003.
We are actively involved in the Quality and Outcome Framework and have signed up to a whole range of additional National, Local and Direct Enhanced Services.

Their role is to ensure that patients enjoy a healthy and trouble free pregnancy and have regular clinical monitoring and advice.
Health visitors are based at Child Health Centres but they also work with the GP practice to support families.
Post-natal care
This service is provided by the practice team, involving the Doctor, and nurse. Their role is to ensure that mother and baby receive the post natal care they deserve and to answer any queries they may have. The health visitors also provide a vital role in this period for both mother and child with advice and regular monitoring up to the age of 5 years. We also provide help via referrals to Child Health Services at NELCTP.
The team give continuing care with Post-natal examination and follow-up. The midwife visits at home and provides maternal and neonatal clinical care, support and advice. The Health Visitor is available to provide holistic maternal and family support and to advise on the physical health and development of the children.
Community Midwife: The Midwife sees patients at the nearest Child Centre to our surgeries by referral from the Practice. They also attend patients in their home after delivery. They give routine screening advice and will refer patients to the maternity hospital for a 12 week scan.
Health Visitor: Post-natal home visiting is available to parents from the 12th day after delivery. Development Assessments and Health Promotion advice is offered to all children at the age of 6 weeks, 7 months, 24 months and 42 months.
Doctor: The doctor is available to all ante-natal and post-natal parents if they have any problems. An appointment can be made in the normal way either by telephoning or calling into the surgeries. Post-natal examinations are completed by the doctor as required by making a routine appointment via the receptionist.

What we do
We loan out a wide range of equipment from walking sticks/frames, bath hoists and shower chairs to grabbers, specialist cushions, slide sheets and commodes. These specialist items are aids for daily living which can make a real difference to your life by helping you maintain your independence and ultimately stay in your own home for longer.

Equipment can be requested by district nurses, occupational therapists or physiotherapists and is provided either by the NHS or via social services funding.

Breast Imaging
Our breast imaging and diagnostic service is run from our dedicated unit called the Pink Rose Suite which is based at Grimsby hospital.
Breast cancer is the most common cancer in the UK, with around 55,000 new patients diagnosed every year.
Treatment for breast cancer is most effective when found and diagnosed early. The NHS Breast Screening Programme aims to detect breast cancer at a very early stage.
What we do
Our breast imaging and diagnostic service is run from our dedicated unit called the Pink Rose Suite which is based at Grimsby hospital.
We are part of the Humberside Breast Screening Service.
We have two dedicated digital mammography rooms capable of standard mammography, tomosynthesis and stereotactic (computer) guided breast procedures. In addition there are two ultrasound rooms with up to date equipment aiding accurate diagnosis and guiding invasive procedures.
Our staff are specialists trained in breast procedures providing a friendly, knowledgeable and accurate diagnosis as part of triple assessment with the surgical team.

Cardiology Services
What we do
We provide diagnosis and treatment for a range of heart problems including arrythmia, congenital and ischaemic heart disease, heart failure, hypertension and valve disorders.
We have coronary care beds at Scunthorpe and Grimsby hospitals in addition to specialist rehabilitation and respiratory teams. We also offer outpatient services at our Goole hospital.
Patient care and well-being is of the utmost importance to our staff and each patient has a named nurse so we can provide individualised nursing care, taking into account each patient’s physical, psychological, spiritual and social needs. All procedures are undertaken with a friendly yet professional attitude to attempt to put patients at ease during what may be a difficult time.
Coronary care beds
Our coronary care beds offer critical care to patients with a range of cardiac conditions including acute myocardial infarction (MI), unstable angina, cardiac rhythm disturbances, and heart failure.
Our patients may have several cardiac investigations during their stay with us including coronary angiography, permanent pacemaker insertion, and implantable loop insertion.
Staff are supported by an excellent cardiac rehabilitation service and also by a heart failure specialist nurse service.
We also have facilities for emergency insertion of temporary pacemakers, NIV – (CPAP Mode), high flow oxygen therapy, emergency DC cardioversion and pericardiocentesis.
Other service that we offer include:

Trans-oesophageal echocardiography
Dobutamine challenge stress test
Myoview scans (Grimsby only)
Percutaneous coronary intervention (PCI) (Scunthorpe only).
If you are coming in as a patient, or you are visiting a loved one, then please be aware we do use a lot of equipment which allows us to give the best possible care. We realise this may seem frightening at first but please do not be alarmed by what you will see.
It is normal for patients to be connected to cardiac monitors which have alarms and may be noisy; this allows us to observe the heart rhythm. Patients may have a needle in their hand, arm or side of their neck; this is to allow us to give drugs quickly. A drip or more than one may be necessary to give medicines to help with their treatment. A face mask may also be used to deliver oxygen, which will help to relieve pain and will also help if they are short of breath.
Outpatient service at Goole
We hold a weekly outpatient clinic at Goole and District Hospital in addition to rapid access chest pain clinics. ECGs are also available.

We provide high levels of patient care across a wide range of surgical procedures that span specialties including colorectal, upper-gastrointestinal, vascular and breast.
General surgery focuses on the abdominal organs, from the intestines, small bowel and colon through to pancreas, gall bladder and more. Our consultants all specialise in their own areas, from colorectal surgery to upper gastro intestinal surgery, but undertake on call rotas for emergency care to non-elective patients for any general surgery complaint.
We are proud to be heavily involved with research projects and have a profound input in the Hull and York Medical School, with many students keen to undertake their colorectal training at our hospitals.

Maxillofacial And Orthodontics
Our maxillofacial team diagnose and treat diseases affecting the mouth, jaws, face and neck.
Our team cares for patients with minor, as well as major, problems affecting the mouth, jaws and face. We are an important referral resource for surrounding dental and medical practitioners. We operate as an arm of Humberside Maxillofacial Consortium with headquarters at Hull Royal Infirmary. Major surgery for mouth cancer and serious maxillofacial injuries is undertaken in Hull where our department has a dedicated ward and a trained nursing team. Most emergency care will be undertaken in Hull with elective surgery lists continuing in our local hospital.
We have an expert consultant-led team able to offer extensive treatment options to all our patients on site. We offer the full range of services as defined by the British Association of Oral and Maxillofacial Surgery. and are also able to provide outpatient and inpatient surgical care for patients with varying underlying medical problems.

Cardiac Rehabilitation
We offer comprehensive and tailor-made programmes of care for our heart patients, which includes education, advice, monitoring and a variety of exercise programmes. This helps them and their families to regain their confidence and resume as near normal a life.
Our patient groups include heart failure, myocardial infarction, cardiac surgery, angioplasty and we also have patients who have had cardiac transplant, angina and internal defibrillators implanted through the service, although these are less routine.
Referrals
Patients are mainly referred from the nursing staff on the wards, but we also take referrals from other hospitals, GP’s, practice nurses and community matrons.
We have an excellent partnership with North Lincolnshire Council, which has enabled a continued exercise programme to be established. This ensures a smooth transition for patients to continue their exercise routine, and at this time partners often join in the exercise. We encourage a range of students and professionals to spend time with us, so they can gain a clear insight as to what the service has to offer and how they are able to use us as a valuable resource.
We recognise the importance of high-quality care and strive to maintain and share good practice; we achieve this through networking with other teams, working closely with our cardiac and stroke network and the NHS Improvement Team.

Infection prevention
While you are a visitor in our hospitals please help us prevent the spread of infection by ensuring you comply with our top tips for visitors:
Do not visit if you are feeling unwell or have an infection such as flu, or are suffering from a heavy cold or diarrhoea and vomiting. If you are unsure telephone and check with the person in charge before you visit
Strictly adhere to visiting times and to the numbers of visitors allowed
Clean your hands before entering and upon leaving the ward or place of care
Do not sit on the bed or use the patients’ toilets
Limit the amount of items brought into the hospital that can cause clutter and make cleaning of the ward difficult
Check what kind of gifts or food you are allowed to bring in
Some wards won’t permit flowers and some may prefer you not to bring food
Do not touch your relative’s/friend’s wound or any devices such as drips or catheters
If you need to take patient clothing home to be laundered you can also help to reduce the risk of infection by following our advice.

Nutrition And Dietetics Department
Nutrition and dietetics is the professional study of nutrition and how it applies medically to health and disease. Our dietitians work with patients with acute and chronic illnesses within our hospitals and out in the community. They use dietary manipulation and food to treat disease and optimise health.
Unlike nutritionists, dietitians must be qualified in their field as a registered HPC and this title is legally protected within the medical profession. Nutritionists or nutritional therapists may offer advice on diet or nutrition however any training or professional practice that they have may be unregulated.
Dietitians have many important roles in health care and our staff work closely with other specialties to provide comprehensive care to patients suffering with illnesses such as, diabetes, obesity, gastrointestinal illnesses, eating disorders and cancer. We also carry out extensive work with the children’s centre to enable healthier eating and cooking skills.
Dietitians provide:
?Nutritional support: using normal food with extra calories and nutrients added or specialist nutrition products to supplement or replace normal food and drinks in malnourished patients. Some may require feeding tubes
?Therapeutic diets: eating more of or the avoidance of certain foods can help to control a patient’s symptoms, improving their quality of life
?Advice on nutritional issues to various committees including the nutrition strategy group
?Education: our staff are committed to educating other health professionals through training.
Hospital service
Every ward has an allocated dietitian responsible for all dietetic services to that area and once patients are referred to a dietitian by a member of ward staff we aim to see you within 24 hours. All of the qualified dieticians wear black or navy trousers or skirts and a white tunic with a light blue stripe on the collar and sleeves. The tunic will also have our light blue nutrition and dietetics logo on it.
General dietetic outpatient clinics are available which include nutritional support, in addition to the specialist areas of paediatrics, diabetes, gastroenterology, weight management, chronic obstructive airways disease (COPD) and oncology.
Community service
We strive to improve the health and wellbeing of people in North East Lincolnshire by promoting healthier eating through practical and initiative interventions. As dedicated dietitians, nutritionist and food workers the team work together to promote healthy eating and lifestyle for all members of the community: babies, children and adults. Their practical, hands on approach and guidance enables people to make informed decisions about their own and their families’ eating habits and attitudes towards food.
We work closely with North East Lincolnshire Council children’s centres and have developed tailored cookery sessions for:
Healthy eating during pregnancy
Promoting Healthy Start
How to wean your baby safely
Helping with those older fussy eaters.
Local children’s centres also support our delivery for our weight management service by enabling us to deliver our balance your lifestyle course. The aim is to help parents manage their weight between or after a pregnancy.

Gynaecology Department
You can be referred to the gynaecology department for a number of reasons including general gynaecological problems such as pain in your stomach or excessive bleeding, or more particular concerns such as fertility problems or suspected cancer.
Services offered include:
Hysteroscopy
Colposcopy
Gynaecology cancer
Infertility
Termination of pregnancy.
Usually women are seen within an outpatient clinic setting where a full medical history and usually an examination will be undertaken. At some specialist clinics, such as post-menopausal bleeding clinic, the doctor will also perform diagnostic tests such as a scan or a hysteroscopy. Full information would be given to you within your appointment letter.
We provide colposcopy clinics where ladies will attend following an irregular smear test, or as a referral from their GP. You may also be referred into our infertility clinic if you are experiencing problems conceiving . We run uro-gynaecology clinicss to assess women with both urological and gynaecological symptoms, such as urinary incontinence.
When you first visit us, this is usually within our gynaecology outpatients department butwe also have a dedicated ward for women being admitted for major operations. This is ward 19 at Scunthorpe General Hospital. For women who do not need to stay in overnight we have a purpose built day surgery unit.
Our early pregnancy assessment unit sees women experiencing pain or bleeding in pregnancy of less than 20 weeks gestation. Women with a history of recurrent miscarriage or previous ectopic/molar pregnances are also seen here.

Had unprotected sex or worried that your usual method of contraception has failed? Emergency Hormonal Contraception (EHC) also known as the "Morning After Pill", is available free from ACT Pharmacies. The sooner it is taken after unprotected sex the more effective it is, but will only work if taken within 72 hours.
The Pharmacy will be able to provide you with information and direct you to other services if you are still concerned, even if it has taken some time to seek advice.

Occupational therapists work with people of all ages to help them to overcome the effects of disability caused by physical or psychological illness, development problems, accident or ageing.
Adult Services
Our teams work across North, North East Lincolnshire and Goole and work closely with other agencies including social enterprise and local councils. Occupational therapy can help if you are having difficulty with everyday tasks, such as dressing or getting to the shops. Our staff can provide help and training with everyday activities and offer advice on adapting your home or workplace to meet your needs, helping you to regain or improve your independence.
Much of their work focuses on the functional implications of illness or disability and may involve assessment and rehabilitation for relevant daily living activities – for example, aspects of personal care and meal preparation. This work may be carried out while someone is in hospital or at their place of residence, following discharge. Your occupational therapist can also advise and recommend equipment, such as mobility aids, wheelchairs and other special devices.

Diabetes And Endocrinology
Diabetes is a lifelong condition which requires input from a number of health professionals including doctors, nurses, podiatrists and dieticians. Our dedicated team all aspire to help patients with diabetes to help themselves. Guidance on self-management is provided through education at every contact with patients in a friendly environment.
Diabetes is a group of conditions characterised by high blood glucose levels that result from defects in the body’s ability to produce and/or use the hormone insulin. When you eat food the body breaks down all the starches and sugars into glucose, which is the basic fuel for the cells in the body. Insulin prevents blood glucose levels from getting too high and controls the amount of energy the body can use from the food and from energy stores in the liver and muscle. There are two main types of diabetes:
Type 1 diabetes develops when the insulin producing cells in the body have been destroyed and the body is unable to produce any insulin. Although Type 1 diabetes can develop at any age it usually occurs in childhood or before the age of forty. Treatment is with insulin, a healthy diet and regular physical activity.
Type 2 diabetes is the most prevalent form of diabetes accounting for 85-95% of all diabetes. In Type 2 diabetes, either there is not enough insulin produced by the body to regulate the blood glucose levels, or the body is unable to use insulin properly (this is called ‘insulin resistance’). Risk factors for developing Type 2 diabetes include having a close family member with Type 2 diabetes, being overweight, central obesity, Gestational Diabetes (GDM), being of South Asian or Afro-Caribbean descent. The treatment for Type 2 diabetes focuses on diet and increased physical activity, with medication both to make the body produce more insulin and to be more sensitive to insulin. At least half of the people with Type 2 diabetes eventually need insulin to help control their diabetes.
Adult Services
A wide range of services are offered, with clinics held at the diabetes centre’s at Scunthorpe and Grimsby hospitals and at the outpatient departments at Grimsby, Scunthorpe and Goole hospitals.
Consultants along with two specialist diabetes nurses, three specialist podiatrists and a dietician provide the diabetes service for outpatient clinics.
Clinics deal with all aspects of diabetes management including glucose control, hypertension control, cholesterol control and obesity management. Renal complications of diabetes including proteinuria and diabetic chronic kidney disease are also managed as part of these clinics.
Conditions treated at diabetic clinics include:
Foot complications including ulcers, perhiperal neuropathy and charcot foot
Insulin pump treatment
Diabetes in pregnancy including endocrine and pre-conception clinics.
We also offer an educational course for patients with type 1 diabetes.
Endocrinology
Endocrine clinics are offered covering all aspects of endocrinology, including:
Thyroid disease (eg thyroid nodular disease, grave’s disease, hypothyroidism),
Pituitary problems (eg acromegaly, cushing’s disease, prolactinomas and other pituitary tumours, short stature)
Adrenal gland problems (eg adrenal nodules including incidentalomas, phaeochromocytoma, conn’s syndrome)
Sexual problems (eg hypogonadism including erectile dysfunction, delayed puberty, polycystic ovarian syndrome, hirsutism)
Metabolic problems (calcium related problems including primary hyperparathyroidism, osteoporosis, vitamin D deficiency) and multiple endocrine neoplasias)
Radioiodine treatment is offered to suitable patients with benign thyroid disease including thyrotoxicosis and subclinical thyrotoxicosis.
At Grimsby’s Diana, Princess of Wales Hospital the diabetes centre is located to the side of the outpatient department with access through the clinic area and an external entrance. At Scunthorpe General Hospital there is a separate building next to the car park at the main entrance on Church Lane. the centre is also linked to the main Hospital by a covered out door walkway.

Some service users might be in a severe mental health crisis and need urgent care and treatment.
Our crisis home treatment team are the front door to NAViGO’s services if you need emergency mental health support.
If you contact us, we will give you a triage mental health assessment at Harrison House to find out how you’re feeling. If you can’t attend Harrison House, we’ll visit you in the community or in your own home to carry out the assessment.
Depending on what support we feel would suit you best, we will arrange for you to get the help you need. This may be a referral on to other services, intensive treatment within your own home or an admission to our inpatient lodges at Harrison House. No matter what it is, we’ll strive to find the support that will stabilise your mental state while being the least restrictive option available.
We’ll make any decisions alongside you, your family and carers, working closely with them to find you the best treatment.

Sequoia Therapeutic Community
The Sequoia Therapeutic Community is a day psychotherapy programme for people with long-term emotional difficulties.
If you have a diagnosis of Emotionally Unstable Personality Disorder (also known as Borderline Personality Disorder), or you believe you might have one, you may be referred to the two-year Sequoia programme.
Therapeutic Communities (TCs) like Sequoia are based on the work of William Tuke, who believed people needed meaningful support and valued roles within society to improve their health and wellbeing.
And we’re one of the best TCs in the country having been accredited by the Community of Communities – the Royal College of Psychiatry’s accreditation programme.
We provide a structured, three-days a week programme and concentrate on your relationship with others, with different activities deliberately designed to help you adjust to life in outside society.
You’ll be introduced to a few different therapies, including Mentalisation Based Therapy (MBT), Dialectical Behaviour Therapy (DBT), art groups, stress management and more!
If you join Sequoia, we need you to be enthusiastic and wanting to work hard on changing your life! We spend a lot of time together as a community, cooking and eating together and supporting each other on our journeys toward feeling well again.

Community Mental Health Team
The way community mental health teams deliver mental health services across North East Lincolnshire has been transformed. We are now integrated with Primary Care Networks (PCNs), which are groups of GP practices.
The changes have put the service user front and centre. They are focused on ensuring their needs are met, with the support required delivered as quickly and effectively as possible.
What are the aims?
We want to better support more people with serious mental health problems — such as psychosis, bipolar disorder, personality disorder, eating disorders, rehabilitation needs and severe depression — within their local community.
We also want to create a system that is accessible and flexible, bringing together the mental health services we provide at NAViGO with GP practices, social care and the voluntary sector.
This creates a single point of access for mental health concerns that focuses on the needs of the individual, improving access to services, service user experience and individual outcomes.

Community Mental Health Team (CMH)
As patients' lives have returned to normal and demand has increased, we have been listening to our patients and adapting our appointment systems to meet the population need as best we can. GP appointments are currently a mix of face-to-face appointments and telephone appointments, with a limited number available for booking up to 14 days in advance and a portion reserved for urgent cases on the day.
Nursing or HCA appointments for all healthcare including chronic disease checks, injections or blood tests are available to book 14 days in advance.
Requests for GP appointments will be Care Navigated by our highly skilled reception team who will provide you with advice about who can help you with your health need; this could be a telephone or Face to Face consultation with the GP, ANP, nurse, HCA pharmacist or another healthcare provider within the community.

The last few years have shown the importance of being outdoors and how it can benefit people’s mental and physical health (something we know more than most here at NAViGO).
On the back of this, the government announced a multi-million pound investment aimed at preventing and tackling mental ill health through green and blue social prescribing.
Let’s be honest, that’s a weird title. But it basically means connecting people with their surroundings through nature or water-based activities that bring a range of physical and mental health benefits.
The Humber and North Yorkshire region (including us, here in good old North East Lincolnshire) is one of seven ‘test and learn’ sites in England who are looking at the impact of such activity. As one of the test sites, we’re looking at how activities can improve mental health as well as reduce health inequalities. Plus how we can develop best practice in making activities accessible and reduce demand on the wider health and social care system.
So what does this mean for you? Well, you’ll find information about a range of activities on our events calendar - from coastal walks to litter picks, community gardening to paddle boarding.
We’re also offering free taster sessions (for activities that usually charge) to service users so if you’ve ever fancied a go at canoeing or scuba diving, let us know and we might be able to help arrange a session for you.

We also offer fast-tracked access to key workers, including NHS staff on the frontline of the coronavirus pandemic.
The 24/7 line is a joint venture between NAViGO, Rethink Mental Illness, North East Lincolnshire Council, Lincolnshire Partnership NHS Trust and Young Minds Matter.

Springboard Rehabilitation Service
There may be times when you need a bit of extra support in taking that step after being in an adult inpatient facility, or to gain better skills to progress towards living independently.
That’s where our Springboard rehabilitation and recovery service comes in.
Simply put, you live with us while we help you build life skills until you’re ready to make the move to live independently on your own.
Based at Hope Court – a 13 apartment complex with gym, garden, activity suites, quiet rooms and communal areas - each individual has their own bedroom, bathroom and kitchen.
Each person is provided with a thorough assessment of their needs, individual support and step-by-step goals to help them recover and make the move to living independently.
We have staff to support you 24 hours a day, 7 days a week and they will provide psychological support, therapeutic, recreational, occupational and creative activities, as well as occupational therapy.
We will make sure you develop personally so that you can go on to live successfully in the local community which is why we’ll help you find accommodation of your own as well as employment and education opportunities.
The dedicated team will also work closely with social services and your family and friends.

Our friendly pharmacy service ensures staff members, service users and carers have safe and effective access to medicines.
We provide accurate and independent information on medicines to keep you well informed. You can access the Choice and Medication portal, where you can find a wealth of information on hundreds of mental health conditions and medications.
As well as this, we provide clinical and dispensing activities for service users across NAViGO’s inpatient and community teams.
We also ensure medicine management resources are used cost-effectively throughout the organisation.
We have one specialist pharmacist, with community pharmacists and technicians based at Lloyds Pharmacy on Littlecoates Road in Grimsby.
You can visit the Choice and Medication portal at any time.
The portal provides a lot of information on medication and mental health conditions. Content is presented in an easy-to-read style, so you don’t have to be a chemist to understand!

Mental Health Liaison Team
Based at Diana, Princess of Wales Hospital
Triage and assessment at Accident & Emergency
If you find yourself in the Accident & Emergency department at the hospital, you might meet our mental health liaison team.
We’re based at Diana, Princess of Wales Hospital (DPoW) in Grimsby and may be involved in your care if DPoW staff are worried about your mental health.
Working in A&E, we offer support if you need medical intervention after considering or acting on thoughts of self-harm or suicide. Alternatively, you might be experiencing a mental health emergency at the hospital.
We also work across other wards to provide triage, assessment and signposting if you have existing medical issues but have developed mental health problems during your admission too. This includes older people diagnosed with, or suspected to be experiencing, dementia or delirium.
After DPoW discharge you, we work with other services and agencies – both inside NAViGO and elsewhere in North East Lincolnshire – to make sure you carry on receiving the care you need to look after your mental health.
Hospital staff will contact the mental health liaison team if they are concerned for your welfare. If you’re at DPoW and would like to speak to the team, please speak to the doctors or nurses looking after you.
The mental health liaison team work 24 hours a day, seven days a week.

Centre4 - Social Prescribing - Thrive.Nel
Social prescribing aims to connect people living in North East Lincs to activities, groups, and services in their community to meet the practical, social and emotional needs that affect their health and wellbeing.
thrive.nel has a team of Link Workers based here at Centre4, who are helping people to find the support they need, so if you’re considering a different way to improve your long-term health, Social Prescribing could be the right prescription for you!
How Does It Work?
We understand that it’s not always easy to do things differently. Working alongside your GP, an experienced Link Worker will spend time getting to know you on a confidential, one-to-one basis. Link Workers are practical and helpful and will not judge.
You can access support for up to 12 months and during this time your Link Worker will help you explore activities you may want to connect with and which could help you better manage your long-term condition and how you look after yourself.

What Support Is Available?

Befriending, counselling and access to other appropriate support to help you manage your condition better
Getting more active – access to walking groups, gym or keep fit classes
Advice about giving up smoking, losing weight and eating a bit more healthily
Advice about benefits, housing, debt and money management.

Assertive Outreach
The assertive outreach team works with an identified group of service users who have severe mental health problems and aren’t currently engaging effectively with mental health services.
We’re flexible, so people who aren’t comfortable accessing services can still receive help and support. We work with you in your own environment, or a place you feel happy to meet, to persuade you to keep connected to mental health support services.
We sometimes use housing departments, police stations, social security offices and inpatient lodges as meeting places.
Our team is made up of nurses, social workers and assistant practitioners. Our low ratio of service users to staff means we can work more closely and more intensively with you – sometimes as often as twice a day if required.
Engagement is the core concept of assertive outreach. It’s a vital component in the development of a therapeutic relationship between NAViGO and people with mental health illnesses.

Psychology
The psychology team provide evidence-based psychological therapies that focus on reducing mental health distress.
You may come across the psychology team as they deliver psychological assessment and treatment for service users across NAViGO. That includes those under the care of the community mental health team , Rharian Fields specialist eating disorder unit , Springboard rehabilitation and older people’s services .
We help people manage severe mental illness through different treatment options. After giving you a thorough assessment of your individual needs, we’ll identify the most appropriate therapy and number of sessions for you.
Psychology may introduce you to any of a number of therapies including:
Cognitive Behavioural Therapy (CBT)
Eye Movement Desensitisation and Reprocessing (EMDR)
Dialectical Behavioural Therapy (DBT)
We’ll work alongside you and tailor therapy to your needs, taking into consideration factors like your personality, history and current presentation.
If you aren’t currently a NAViGO service user, but believe your mental health would benefit from talking therapies, you should contact Open Minds in the first instance.

Speech and Language
We support children and young people from birth to 17 years with Speech, Language and Communication needs. For most children this will mean help developing their spoken communication. For other children it may include using visual communication strategies, for example: Makaton signing, use of pictures or objects.
Children aged three to 17 years old can be referred by parents/carers, teachers, GPs or other professionals. Parents are recommended to speak to their child’s school or health visitor if they are concerned about their speech and language development. Children with feeding and/or swallowing problems need to be referred by their doctor.

Occupational Therapy and Physiotherapy
Occupational Therapy and Physiotherapy within North East Lincolnshire helps children and young people to participate as fully as possible in daily activities.
Occupational Therapists help children and young people to carry out the things they need or want to do in areas of self-care, school, work and play. They work with children and young people between the ages of 0 and 19 years who have difficulties or a disability, which affect their ability to carry out everyday tasks.
Children’s Physiotherapists are responsible for the assessment and physical rehabilitation of children needing treatment due to disability, accident, injury or disease. Children’s physiotherapists have generic skills as physiotherapists and also additional knowledge and experience of child development and of childhood conditions.
Treatment is based upon an assessment of the child’s needs and the formation of individual treatment programmes. The physiotherapists use a range of approaches that are individually tailored to each child’s needs. This may include exercise programmes, postural management, mobility aids and other specialist equipment.

Dietitians
Childrens dietitians are the experts in nutritional care of infants, children and adolescents. We can assess your child’s diet and medical history and work with you to decide on a plan to support your child’s individual nutrition needs.
We support babies, children and young people, together with their families and carers across North Lincolnshire and North East Lincolnshire to optimise their nutritional intake and support healthy growth and development. We see patients and families in a variety of settings including outpatient clinics, hospital wards and special schools.

Children’s Continence Service
The Children’s Continence service is a specialist service that offers advice and support to children and young people within North East Lincolnshire who may have problems with their bowel or bladder. The types of problems you may get referred to the service with may include:

soiling
toilet training difficulties
assessment for continence products if deemed appropriate (from the age of 5 years old)
The service supports all children and young people, from the age of 5-18 years old with a bowel or bladder problem.
