Introduction to the Service:
St Andrew?s Children?s Hospice provides palliative care in a friendly home-from-home environment for children and young people from birth to 25 years of age who live in the Lincolnshire and Hull & East Riding areas.
What our service does:
We provide specialist palliative care, regular short breaks for the child/young person or for the whole family together. We also provide symptom control, emergency and end of life care, specialist advice and expertise, practical help and information as well as 24-hour telephone support. Alongside this we have a team, which offers family support, complementary therapies, chaplaincy and creativity services. We also support brothers and sisters of anyone accessing the children?s hospice.
What is the purpose of our service?
St Andrew?s Children?s Hospice is here to provide support to the children/young people and their families through the challenges that having a life-limiting condition can bring over many months and years. Support For Care Leavers (16+)

Through Care
Through Care provides services to young people and children who are in long term care or have left the care of the local authority.
Children can be looked after in two ways. They are mainly either looked after with the agreement of their parents under Section 20 of the Children Act or by order of the court under a Care Order. The young people need and deserve the most positive experiences from their corporate parenting. We seek to promote good health and education provision and support to develop their resilience and improving outcomes. Stability of placement and support are crucial to achieving good outcomes.
Through Care place the child at the centre of their work, they: •Strive to make care planning effective •Ensure young people have accommodation that meets their needs •Review the care plan in a timely manner
The Local Authority has produced a Pledge document with the help of Looked After Children. The Pledge sets out the key promises from the Local Authority to the Looked After Children and Care Leavers.
Preparing for independence
Young people will leave care either on or before their 18th birthday depending on their needs. Young people will be involved in the decision making about preparing them to leave care. This process will start before the young people leaves school and will look at what they need to happen before they are ready to be independent. This process Support for care leavers (16+) It is important that you know what support is available when you become a care leaver.
Support for care leavers To access care leavers support, you must have been in care for at least 13 weeks (this does not have to be 13 weeks in a row) after age 14 and included some time after your 16th birthday.
If you are not sure whether you qualify for support, or have any questions please ask your Social Worker or Personal Advisor.
We will encourage you to stay in care until you are 18. Most young people still live at home with their families at this age.
We must provide you with suitable accommodation options up to the age of 18.
Personal advisors All care leavers are entitled to a personal advisor, which is legal term for a leaving care worker. An advisor will be allocated between your 16th and 18th birthdays, depending on your circumstances. Your personal advisor will in many ways be the guide as you leave our care. Personal advisors are not social workers but they have lots of other experience and qualifications.
Your personal advisor is there to:
listen to you and help you get ready for the next steps make sure that decisions are made that are best for you help you to prepare to live independently offer advice and support after you leave care record this information in your pathway plan a visit to yourself will be completed a minimum of 6 weekly The amount of support that you receive from your personal advisor will depend on what you want and your circumstances.
We are based at the Civic Offices in Cleethorpes where we have a special room designed with young people so that you can talk with your worker in privacy and comfort.
We will try our best for you to keep the same worker during your time with the service. Each year when we ask our care leavers they talk about how important their worker is to them. Having consistency is important.
We also recognise the need to have a range of workers because not all styles suit all young people.
Should you at any point wish to change your worker we will speak to you about the reason why you want to change and if it is agreed that this is best for you we will consider this. However it is not automatic.
We will actively stay involved until you reach the age of 21 years old. We will continue to support you post 21 if there is a specific need. You can request support at any time up until your 25th birthday.
What kind of help do you get? Here are some examples of ways in which we help you from the age of 16 onward:
helping you to plan for your future and achieve your ambitions help with finding the right education and training opportunities after you leave school (we can help you with any extra costs or equipment you might need for courses) finding work experience opportunities support to apply for apprenticeships help you to find the right sort of accommodation at a time that is right for you (for example remaining with your former foster carer at 18 in a supported lodgings arrangement) help with the costs of setting up in your own flat (setting up home allowance) help you to keep in touch with family, carers and friends and develop good support networks wherever you are living make sure you have the chance to develop your independent living skills (for example cooking, cleaning, managing money and paying bills)

Careers, education, training and employment Young People’s Support Services (YPSS), based at the Molson Centre Grimsby (Thesiger Street entrance – the side door where the green railings are) supports young people aged 13-19 (or 25 for those with additional needs) with information, advice and guidance about careers, further education, training and employment.
School age young people Individual schools/academies are responsible for arranging independent and impartial careers guidance for the majority of students however YPSS will arrange to see some students between years 9 and 11 depending on their individual circumstances, for example young people with an Education Health Care Plan or children who are looked after by the local authority. If you have not met your careers adviser in school, you don’t attend school or would like to organise a careers appointment outside of school please contact YPSS. A careers interview can be undertaken in a variety of ways at a variety of venues, for example 1:1 appointments, telephone calls or through Microsoft teams meetings.
After you leave school YPSS have a team of NEET Practitioners who support unemployed young people aged 16-18 (up to 25 if you have an active Education Health Care Plan or are/have been Looked After by the Local Authority) with finding and moving into suitable provision.
Making important decisions about your future can be difficult and if the plans that you had have not worked out it can be hard to think about what other options you may have. Or, if you have a problem that is preventing you from getting where you want to be it can be hard to know where to find support. This is where YPSS NEET Practitioners can help.

SEND Outreach If you need extra support with physical development you need to speak to your school Special Educational Needs Co-ordinator (SENCO) or the SEND Outreach Team.
About the team The SEND Outreach Team is made up of Project Workers who work on physical development, including your ability to make movements using the both small and large muscles in your body and your handwriting.
If your school has concerns about you or your physical development, the SENCO may ask someone from the SEND Outreach Team to become involved with you. If this happens, they will work with you, your parents or carers and your teachers, to find ways of helping you at school.
How we can help you Here are some of the ways they can help you, in school:

Digital Inclusion is a cross-sector collaboration with a number of organisations working together to create a digitally inclusive borough. We want to make sure no one is left behind as digital becomes the way to access services, communicate with organisations and interact with friends and family
The Digital Inclusion project forms part of our wider vision for a better more equal borough that supports and enables residents to improve their lives. We want to make sure that residents have the capability to use the internet to do things that benefit them day to day. We aim to ensure our residents have access to the kit, connectivity and are equipped with the skills they need to make the most of digital opportunities.
To help achieve this we are developing a Digital Network of digital partners and providers from across the public sector, voluntary and community sector as well as business partners. This network will serve to jointly tackle digital exclusion and will work collaboratively to find solutions for our residents.
Our aim Our aim is to enable everyone who wants to be digitally enabled, where our residents have the confidence, skills and access to the internet they need to gain the work, health and life benefits the internet provides.
Why digital inclusion matters With the world around us becoming increasingly digital, the access and capacity to use online services is a necessity. We are committed to digital accessibility and are determined to get our digitally excluded residents safely online to fix the digital divide.
Residents will not be left behind. Digital exclusion can have serious negative impacts on a person’s job prospects, education, health and wellbeing and finances. Through not having the digital skills and access to utilise the online world an individual is at an increased risk of falling into poverty.
We are committed to making sure this doesn’t happen anymore, with our team of dedicated partners ranging from businesses, community groups and charities we are focused on providing the solutions and access to devices, digital connectivity and skills to ensure no one is left behind.

Further Education SEN provision Cambridge Park Sixth form offer a small and supportive environment with access to entry level qualifications in functional skills (English and maths) alongside experience in several vocational areas on a rotation including catering, hair and beauty, horticulture and various crafts. Students also develop independent living skills as well as taking part in ongoing work based learning linked to their vocational courses and take part in formal work experience placements. Grimsby Institute offer a number of programmes at foundation (entry) level which include Pathways to Employment, Preparation for Adulthood, life skills, functional skills and tasters in various subject areas. Transition Planning. Moving to the next stage of education Once new school places are confirmed, families start thinking about the transition, from practical ones about drop-off and pick-up, uniform and homework, to concerns about their child making friends or settling with their new teacher.
In North East Lincolnshire parents, carers and their children will be invited into educational settings for information sessions and for children to meet their teacher. This will help them to get a feel for the school, the staff and the way things run in new settings, and what parents can do with children while they’re at home to get them ready for transition. Seeing the school building and layout can be really useful as can virtual tours on school websites.

What is Independent Travel Training? Independent Travel Training (ITT) is a free scheme for young people aged 14 years and over who are currently in full-time education and are in receipt of local authority transport support. It will help them to travel independently between college/school and home.
ITT encourages children and young people with special educational needs to reach their potential.
ITT is a free service for children and young people who have Special Educational Needs and Disabilities.
The ITT team work with young people through referrals from schools, colleges, and parent/carers.
What are the benefits of Independent Travel Training? Increased independence Increased self-esteem and confidence Improved social skills Independent travel skills that could be developed and applied to other activites Increased access to leisure and social events Access to other study or work opportunities
What support will be given? The travel training programme will provide students with a wide range of support which will include:

Parents and carers are legally responsible for ensuring that a child attends school. However, in some cases the local authority has a duty to support parents and carers with transport to school.
Secondary school children from low income families the child is entitled to free school meals or the parent/carer is in receipt of the maximum level of Working Tax Credit the school is one of the three nearest schools to the home address the distance between home and school is more than 2 miles but less than 6 miles Eligible children are entitled to a Stagecoach bus pass which will allow the child to travel to and from school, free of charge, on Stagecoach service bus routes (Subject to terms and conditions).

Primary and secondary-aged children attending their catchment school the school is more than 3 miles from the home address or the route when walking (accompanied where necessary by a responsible person) is deemed to be hazardous there is no bus service route This normally applies to rural areas.
Eligible children are entitled to a Statutory travel pass which allows children to travel to and from their catchment school, free of charge, on contracted bus routes.

Children with special educational needs and/or disabilities (SEND) Children and young people who have either a statement of special education needs or an Education, Health and Care Plan (EHCP), who are unable to walk to school, being accompanied by a responsible adult or not, may be entitled to free school transport.
They may qualify for support to a school other than the catchment school, for example to a special needs school and/or where the distance is less than the normal qualifying distance. Each case will be considered on the grounds of the individual child’s or young person’s needs and abilities.
An initial decision regarding the individual’s entitlement to transport support in principle will be made during the EHCP process and the transport application form sent to the parent/carer for completion.

Young people with special educational needs and/or disabilities (SEND) in post-16 education Young people in North East Lincolnshire who have an Education, Health and Care Plan (EHCP) or a medical/physical disability and are unable to travel independently may be considered for transport support up to the age of 25 years or until they have achieved their educational goals as identified in the EHCP, where appropriate.
In order to be considered for support the young person must be attending a full-time course of education. Post-16 transport applications are available from the Education Transport team from May/June each year. Further information is available in the Post-16 Transport Policy.
Independent travel training (ITT) Independent travel trainers support and train young people to travel independently to and from their place of education. The training is primarily intended for young people currently in post-16 education, or in the final two years of their statutory school education (years 10 and 11) who are in receipt of local authority transport support to their school or college.
Referrals may be completed and submitted by any party involved with the young person, such as parent or carer, the school or college attended, transition workers and/or a careers adviser with the Young Peoples Support Services (YPSS).
