Child death reviews


The responsibility for ensuring child death reviews are carried out is held by ‘child death review partners’, who are defined as the local authority and any clinical commissioning groups operating in the local authority area.

The Child death review partners must review all deaths of children resident in the local area. They can also review deaths of children who don’t live there if they think it’s necessary. The purpose of these reviews is to find out if there are any issues related to the deaths that affect children’s welfare or public health and safety, and to decide if any actions are needed to prevent more deaths.

New arrangements for the review of all child deaths are being introduced by end of September 2019.

Review partners

Child death review partners from two or more local authority areas can join together and treat their areas as one for reviewing child deaths. North Lincolnshire CCG and North Lincolnshire Council have agreed to work with North East Lincolnshire CCG and North East Lincolnshire Council to review child deaths in both areas through the Northern Lincolnshire Child Death Review arrangements. Additionally, these arrangements will also help share learning with East Riding of Yorkshire, Hull, North Yorkshire, and York through a collaborative approach.
