Team Around the Family (TAF)

TAF aims to support children and family at the earliest opportunity. It refers to different people and services that will work together to prevent or reduce difficulties for children and their families.

There will be a lead professional who works with children and knows your child. Working together families and the lead professional will invite other professionals to work with and support the family.

Family Hubs

Your local Family Hub brings together support from services including SureStart Children’s Centres, Health Visiting, School Nursing, Families First Practitioners, Early Help and some young people services. They work with partners in the community across North East Lincolnshire to identify and offer services for children, young people, and families from pre-birth to adulthood. Locality Family Hubs are for everyone, including support for children with additional needs.

Integrated Front Door (IFD)

Our Integrated Front door is available to all partners and public if they are worried or concerned regarding Children. We can provide advice and guidance and ensure appropriate action is taken where safeguarding needs are identified. Where completion of the Early Help assessment (Word, 65KB) is appropriate, the TAF team are able to support with advice.

If you are concerned that a child is being abused, or they may be at risk of harm, you must contact the IFD – 01472 326292 or go to report a concern about a child.

If a child is in immediate danger call 999.

Professionals who have concerns or worries around a child or family, that are not of a safeguarding nature, should complete an Early Help assessment (Word, 65KB) .

Professionals can use the Early Help assessment (Word, 65KB) to request a service or single piece of intervention from the Locality Family Hub Offer (PDF, 143KB) , and the Helping, supporting and protecting children and families in North East Lincolnshire threshold document 2024/28 (PDF, 614KB) (this documents identifies the crossing points between early help, safeguarding and child protection).

As a professional, if you have concerns around a child or family, where you believe they need a Multi-Agency response and/or more than a single piece of intervention, you must complete the Early Help assessment (Word, 65KB) with the family.

After you have completed the Early Help assessment and have the family’s consent, send the information to [email protected].

Relationships Matter

Relationship Matters helps families who may be having relationship difficulties.

Whether couples are together or separated, the way they communication can impact on their relationship and their child.

Relationships Matter can help parents or carers build healthy relationships, and ways to communicate that put their children’s needs first.

Neurodevelopment pathway

The Neurodevelopment pathway has been moved to NAViGO’s Neurodevelopmental Service. This was to reduce waiting times and streamline children and young people’s mental health. The services takes referrals for children and young people who need specialist autism or attention deficit hyperactivity disorder (ADHD) assessment.
